Written in VB.Net I have recently updated the Massey Speech Project. This project enables you to use speech commands to control x-10 devices
,media player, read the news (RSS feeds) ,set alarms and so on. Available only for Windows Vista.

It can also use Microsoft agents as shown on the web page above. An XP version is also available but not nearly as good. The only thing you may need to buy is a Microsoft Agent. It will work without them however.
