I have made a replacement insert for the HR12A, and am making it available freely available here for non-commercial purposes. It splits the pane vertically to allow boxes to label both the 1-8 side as well as the 9-16 side. It is at 300 lpi resolution and prints out cleanly on both postscript and non-post script printers capable of rendering 6 point text without difficulty.
It comes in two flavors - one that is a regular flat .JPG file that one can edit in any photo/text editor.
The second is for users of Photoshop, and is an uncompressed photoshop file with 18 editable layers with place text boxes already available.
To work properly after printing, you should cut the insert so as to *remove* the outer black border. The circle for switching home codes can easily be removed using an old fashioned hand held paper punch.
